MODERATE — Monitor Closely
CAUTION — monitor for depression/anxiety breakthrough
Evidence level: MODERATE
Rifampin may reduce the effectiveness of Zoloft, worsening depression or anxiety.
Rifampin induces CYP3A4 and CYP2C19. Sertraline is partially metabolized by these enzymes. May reduce sertraline levels.
Monitor mood closely during rifampin therapy. SSRI dose may need adjustment. Inform both prescribers.
Take Sertraline and Rifampin at their usual prescribed times. Some antibiotics (linezolid especially) have serotonergic properties and should never be combined with SSRIs — if you are on linezolid, confirm this combination with your prescriber. Standard antibiotics like amoxicillin or azithromycin are generally safe alongside SSRIs.
Active depression, recent dose stabilization
Worsening depression, anxiety, irritability, insomnia
Inform both doctors. Monitor mood closely during TB treatment.
Discuss this combination with your prescriber. They may adjust timing, dosing, or switch to an alternative with fewer interaction risks. Never stop an antidepressant abruptly — taper under medical guidance.
