DXM and tramadol together double the serotonin syndrome risk and add opioid sedation.
Both are serotonergic. DXM inhibits serotonin reuptake; tramadol is an SNRI-like opioid. Combined: high serotonin syndrome risk plus additive CNS depression.
Never combine. If you need cough suppression while on tramadol, ask your doctor.
Take Tramadol exactly as prescribed — set alarms for scheduled doses. Dextromethorphan follows its normal schedule. Opioids slow GI transit, which can affect absorption of other medications. Take with food if nausea occurs. Never combine with alcohol.
