FDA-documented interaction. Dual mechanism: pharmacokinetic (CYP2D6 inhibition) and pharmacodynamic (serotonergic). High risk of TCA toxicity and serotonin syndrome.

Prozac dramatically increases clomipramine levels in your blood AND both drugs increase serotonin. This creates a double danger: tricyclic antidepressant toxicity (heart rhythm problems, seizures) and serotonin syndrome.
Fluoxetine is a potent CYP2D6 inhibitor. Clomipramine is metabolized by CYP2D6. Increased clomipramine and desmethylclomipramine levels. Also additive serotonergic effect (both are serotonin reuptake inhibitors). Dual risk: TCA toxicity AND serotonin syndrome.
This combination requires very careful medical supervision. If prescribed both, clomipramine dose must be reduced and both cardiac and serotonergic symptoms monitored.
Take Fluoxetine at the same time each day — morning is most common to avoid sleep disruption. Clomipramine follows its prescribed schedule. SSRIs have a long half-life, so consistency matters more than the exact hour.
Higher risk for: elderly, those on multiple serotonergic drugs, people with liver impairment, CYP2D6 poor metabolizers, recent dose changes, or concurrent use of MAOIs or triptans.
