Cymbalta (duloxetine) with tramadol increases your risk of serotonin syndrome. Cymbalta also slows how your body processes tramadol, which can make the interaction even more unpredictable.
Serotonin syndrome: duloxetine (SNRI) plus tramadol (serotonin reuptake inhibitor). FDA labels warn. Duloxetine also inhibits CYP2D6, affecting tramadol metabolism.
Tell your doctor about both medications. Duloxetine is often used for both depression and pain — it may be possible to rely on duloxetine alone and avoid tramadol.
Serotonin syndrome risk exists with some opioids (tramadol, fentanyl, meperidine) combined with SNRIs. Take each at prescribed times. Report agitation, tremor, rapid heartbeat, or muscle rigidity immediately. Morphine and oxycodone are generally lower serotonin risk.
Higher risk for: elderly, those on multiple serotonergic drugs, people with liver impairment, CYP2D6 poor metabolizers, recent dose changes, or concurrent use of MAOIs or triptans.
