FDA label recommends atorvastatin dose should not exceed 20mg daily when used with strong CYP3A4 inhibitors like clarithromycin.

Clarithromycin significantly increases atorvastatin (Lipitor) levels in your blood. While less risky than with simvastatin, this still raises your chance of muscle damage.
Clarithromycin (strong CYP3A4 inhibitor) increases atorvastatin exposure. Atorvastatin is partially metabolized by CYP3A4. FDA atorvastatin label recommends dose limit of 20mg with strong CYP3A4 inhibitors.
If you take more than 20mg of Lipitor daily, your dose should be reduced while taking clarithromycin. Tell your prescribing doctor about this interaction.
Some antibiotics (erythromycin, clarithromycin) strongly inhibit CYP3A4 and can dramatically increase statin levels, raising rhabdomyolysis risk. Take Atorvastatin in the evening as usual. Your prescriber may temporarily pause or reduce the statin during short antibiotic courses.
Higher risk for: those with liver impairment, genetic CYP enzyme variations (poor or ultra-rapid metabolizers), elderly, those on multiple CYP3A4 inhibitors, high-dose statin therapy, or history of myopathy.
Muscle pain or weakness, dark urine, unexplained fatigue
Request atorvastatin dose reduction to 20mg maximum during clarithromycin course. Consider azithromycin as alternative antibiotic.
Statin interactions often involve liver enzyme competition. Ask your doctor about alternative statins (rosuvastatin and pravastatin have fewer CYP interactions) or spacing doses 12 hours apart. CoQ10 supplementation may help with statin-related muscle discomfort.
